Your Opportunity
Join the Acute Care Team within the Hunter Valley Lower Mid North Coast Mental Health Service delivering high-quality outpatient mental health care across the Hunter Valley and Lower Mid North Coast regions. The team provides consultation assessment treatment and follow-up for adults and their families/carers with a strong focus on recovery hospital-to-home transitions medication monitoring psychoeducation and relapse prevention.
As a Mental Health Clinician Acute Care Team you will provide discipline-specific interventions consumer-centered care and care coordination for clients experiencing mental health concerns. You will work collaboratively within a dynamic multidisciplinary team including nurses allied health professionals and administration staff to ensure seamless person-centered continuity of care.
This position operates across a 7-day rotating roster with shifts between 8:00am and 8:30pm.

Key Responsibilities

  • Provide comprehensive mental health assessments develop care plans and implement evidence-based interventions for clients and their families/carers.

  • Deliver consumer-focused family-inclusive care in line with professional legislative and organisational standards.

  • Collaborate effectively with GPs CMOs and other key stakeholders to ensure coordinated high-quality care.

  • Independently manage client caseloads develop rehabilitation plans and monitor progress to achieve optimal outcomes.

  • Maintain accurate and timely clinical documentation in accordance with MH-OAT and NSW Health requirements.

  • Plan and deliver education health promotion and prevention initiatives for clients families colleagues and community groups.

  • Advocate for the needs and rights of clients and their families/carers within the healthcare system.

  • Contribute to service improvement initiatives research clinical audits and program evaluation activities.

  • Demonstrate and promote workplace behaviours consistent with the HNE Health Values Charter NSW Health Code of Conduct and Closing the Gap strategy.

  • Identify assess and manage workplace hazards ensuring compliance with NSW Health Work Health and Safety policies.


About You
You are a motivated and enthusiastic mental health professional with experience in providing holistic care within a multidisciplinary team. You have the ability to build rapport with consumers provide recovery-focused care and contribute effectively to a dynamic outpatient service. You are confident working in a complex community-focused environment and are committed to professional growth and continuous learning.

Essential Criteria

Tertiary qualification in Occupational Therapy Psychology Nursing Social Work Counselling Nutrition or Dietetics with eligibility for relevant professional association membership and/or AHPRA registration.

  • Eligibility to drive in NSW and willingness to travel as required.

  • Clinical experience in rehabilitation including work with children and young people particularly post-acquired brain injury.

  • Strong communication negotiation and conflict resolution skills with clients carers health professionals and funding bodies.

  • Ability to work effectively within an interdisciplinary team.

  • High-level computer literacy and experience maintaining accurate electronic records.

  • Experience developing and implementing rehabilitation plans and independently managing client caseloads.

  • Approved iCare/LTCS Case Manager status or willingness to obtain with knowledge of NSW CTP WorkCover and iCare/LTCS.
