Qualifications:

• Education:Masters Degree in Social Work (MSW)
• Licensure:Active NY State license
• Experience:Minimum of 1 year of experience
• Technical Skills:Proficiency with electronic health records (EHR).
• Soft Skills:Empathy cultural competence and a patient-centered approach.

Key Responsibilities:

• Conduct comprehensive psychosocial assessments and diagnoses for patients and their families.
• Provide evidence-based individual group and family therapy.
• Develop implement and monitor individualized treatment plans in collaboration with a multidisciplinary healthcare team.
• Perform crisis intervention and risk assessments implementing appropriate safety plans as needed.
• Advocate for patients and families to ensure access to community resources social services and financial assistance programs.
• Provide care coordination including patient education discharge planning and referrals to appropriate follow-up care.
• Maintain timely accurate and confidential patient records in compliance with all state federal and organizational policies.
• Participate in case conferences team meetings and ongoing professional development opportunities.
