Job Summary

Job Title: Licensed Practical Nurse (LPN) Full Time (Days/Evenings)

Department: ElderOne

Location: This position supports both the Sisters of Mercy (1437 Blossom Road Rochester NY 14610) and Sisters of Saint Joseph (150 French Road
Rochester NY 14618) motherhouses.

Hours Per Week: 40
Schedule: Days/Evenings 7:00am-3:30pm and 3:00pm-11:30pm Every other weekend required

SUMMARY
The Licensed Practical Nurse is responsible for providing direct patient care usually under the direction of a Registered Nurse in accordance with established plans.

This position has a designated home base facility and may require occasional travel to additional locations. Typically travel is limited to one to two locations per day with up to four sites in Rochester NY and one in Newark NY. Mileage reimbursement is provided.

ATTRIBUTES

  • Strong communication customer service and interpersonal skills highly desired.

  • 1 year of experience working with a frail or elderly population preferred.

RESPONSIBILITIES

  • Conducts and documents a thorough observation of each patients medical status upon admission and throughout the patients course of treatment.
    Assists in the implementation of the patients treatment plan under the direction of the RN.
    Provides routine nursing services for patients as directed by the RN.
    Complies with the evaluation treatment and documentation guidelines of the department.
    Completes required documentation in an accurate and timely manner.
    Provides regular patient status updates to the appropriate personnel.
    Attends and participates in department/facility meetings as required.
    Acts in compliance with hospital regulatory and professional standards and policies.
    Maintains clinical competency by pursuing continuing education opportunities.
    Performs other duties as assigned.


REQUIRED LICENSURE / CERTIFICATION SKILLS

  • Graduate from an accredited School of Practical Nursing.

  • Current LPN licensure in State of New York.

  • Basic Life Support (BLS) Certification from the American Heart Association (AHA).

PREFERRED QUALIFICATIONS

  • 1 year of experience working with a frail or elderly population preferred.

PHYSICAL REQUIREMENTS:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally and/or 10 to 25 pounds of force frequently and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking standing or squatting.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience relevant qualifications specialty internal equity location and contracts.
