Your Role as a Home Health PTA (Physical Therapist Assistant)
As a Physical Therapist Assistant in home health you will deliver skilled therapy services under the supervision of a licensed Physical Therapist focusing on rehabilitation fall prevention and functional mobility.
Key Responsibilities:
- Provide in-home physical therapy services following the established Plan of Care
- Assist patients with therapeutic exercises mobility training and rehabilitation programs
- Support senior care patients in improving strength balance and independence
- Monitor document and report patient progress to the supervising Physical Therapist
- Educate patients and caregivers on home exercise programs safety and fall prevention
- Collaborate with physicians RNs and the interdisciplinary care team
- Complete documentation using WellSky Kinnser EMR
Qualifications:
- Active California Physical Therapist Assistant (PTA) license
- Current CPR certification (AHA required)
- Minimum 1 year of clinical experience (home health experience preferred)
- Strong interest in home health rehab and senior care
- Excellent communication and organizational skills
- Valid drivers license auto insurance and reliable transportation
