NDIS Support Coordinator
Job Summary
We are seeking a passionate and experienced Support Coordinator to join our growing Disability Services team on a part-time basis working four days a week.
About the Role
Youll work alongside NDIS participants to build their capacity strengthen their independence and support them to navigate and maximise their NDIS plans; providing coordination across informal community and funded supports ensuring participants achieve their individual goals while maintaining choice and control. This is a rewarding opportunity for someone who is clientcentred organised and outcomesfocused.
What Youll Bring
- Experience in NDIS Support Coordination disability services or a related human services role
- Strong understanding of the NDIS framework supports and service pathways
- Excellent communication relationshipbuilding and problemsolving skills
- Ability to manage a caseload prioritise tasks and work autonomously
- High level of professionalism integrity and empathy
- Relevant qualification in Community Services Disability Social Work or similar (desirable)
- Current drivers licence NDIS Worker Screening Check and Working with Children Check (or willingness to obtain)
