RN Case Manager Hospice

Triniti Home Health & Hospice is hiring an experienced hospice Registered Nurse (RN) in the role of a RNCase Manager Hospice. Your skills compassion and commitment will be valued in our supportive mission-driven organization.

The idealRN Case Manager (RNCM)is someone whose dedicated clinical expertise results in terminally ill patients receiving exceptional end-of-life this role you will collaboratively works alongside an interdisciplinaryteam to provide end-of-life care to those needing hospice services in their home or community setting. You will provide individualized case management admission facilitation clinical support continuous care after hours call and/or other skilled nursing expertise with chronic and end-of-life care.

Position Type:Full-time benefits eligible position

Wage Range:$40 - $46 / hour depending on experience

Location:1111 Highway 25 Buffalo

RN Case Manager Responsibilities:

• Succinctly convey patient assessments with clinical data to hospice physician who will then determine hospice eligibility
• Provide direct patient care to terminally ill patients; assess identify and address their individual physical psychosocial safety and comfort needs
• Anticipate prevent and treat undesirable symptoms or secondary symptoms
• Develop maintain and manage an individualized care plan that captures the evolving needs of the terminally ill patient
• Train instruct and support the patient family and caregiver
• Document patient problems nursing assessments patient goals care provided and patient/family outcomes from interventions and care provided in a timely manner
• Consult and collaborate respectfully with the interdisciplinary team and others involved in the patients care
• Prepare and present all needed information for bi-weekly IDG meetings

• Graduate of accredited school of nursing
• Current state of Minnesota RN licensure
• Three years recent nursing experience in acute long-term care or home care/hospice/public health
• Patient care planning and patient teaching skills
• Critical thinker with excellent written and verbal communication skills
• Current Minnesota drivers license insurance and daily access to reliable vehicle
