Role Overview
The Resident Care Coordinator provides case management care coordination and behavioral support for adults with developmental and intellectual disabilities. This role focuses on resident transitions retention and holistic care planning partnering closely with admissions caregiving staff and health & wellness teams.
Key Responsibilities
- Coordinate personcentered care plans (CQI)
- Provide case management and behavioral/emotional support
- Support caregivers through training and conflict resolution
- Assist with admissions transitions and discharge planning
- Collaborate on lifeskills classes enrichment programs and activities
- Maintain communication with families and interdisciplinary teams
Qualifications
- Bachelors degree in social work (BSW) Special Education or related field
- 3-5 years experience supporting adults with developmental disabilities preferred
- Background in case management behavioral health or mental health services
- Strong leadership counseling and organizational skills
- Commitment to ethical care and resident advocacy
- Christian faith alignment preferred
Requirements
Safetysensitive position. Must meet state and organizational clearance requirements including: fingerprint clearance background check drug screening CPR/First Aid (or ability to obtain) and APS registry verification.
