Job Summary
Job Title: Case Manager RN
Location: Anchorage AK
Duration: 13 Weeks (Temp to Hire)
Schedule & Shift
- Shift: Day Shift (07:30 AM 04:00 PM)
- Schedule: 5x8 Hours 40 Hours/Week
Position Overview
The Case Manager RN coordinates patient care across the continuum to ensure optimal outcomes appropriate resource utilization and effective discharge planning within an acute care setting.
Key Responsibilities
- Coordinate patient care and discharge planning
- Conduct utilization review and ensure compliance
- Collaborate with interdisciplinary teams
- Educate patients and support care transitions
- Maintain accurate documentation in Epic EMR
Required Qualifications
- Active Alaska RN License (Required at submission)
- Minimum 1 year of RN experience in Acute Care (Required)
- Experience with Epic EMR (Required)
- BLS Certification (Required)
- Strong clinical assessment and critical thinking skills
- Ability to manage multiple cases effectively
Preferred Qualifications
- Case Management experience
- ACM Certification
- Experience in utilization review and discharge planning
- Knowledge of healthcare regulations and payer systems
Education
- Associate or Bachelors Degree in Nursing (Required)
