Job Summary
Position Summary:
The Registered Nurse (RN) in Home Health is responsible for delivering skilled nursing services to patients in their residences. This role focuses on clinical assessment care planning and patient education to support recovery prevent complications and enhance overall quality of life.
Primary Responsibilities:
Conduct thorough initial assessments and routine follow-up visits in the home
Develop implement and revise individualized plans of care
Provide direct nursing care including medication administration wound treatment and symptom management
Observe patient conditions and promptly report significant changes
Complete accurate timely documentation in compliance with regulatory standards
Educate patients and caregivers on health conditions medications and self-care practices
Collaborate with physicians and interdisciplinary teams to coordinate care delivery
Ensure adherence to infection control safety and agency policies
Qualifications:
Current active RN license
Graduate of an accredited nursing program
Current BLS/CPR certification
Prior nursing experience required; home health experience preferred
Strong assessment and clinical decision-making skills
Effective communication and patient teaching abilities
Ability to work independently and manage a flexible schedule
Work Environment:
Care provided in patient homes and community-based settings
Travel required throughout service area
May include evenings weekends or on-call responsibilities
Physical Requirements:
Ability to perform hands-on patient care and mobility assistance
Frequent standing walking bending and lifting
Ability to transport supplies and equipment between visits
