Department: Hospice
Pay Rate: $22.00- $25.00 Hourly (Based off experience)
Hours: Part-time (Hours worked are based on the assigned patients needs)

Summary:
The Chaplain operating within the framework of the mission and philosophy of Hospice provides spiritual religious and personal counseling and guidance to patients family members and employees. Following the interdisciplinary team plan of care the Chaplain organizes and provides counseling and guidance mindful of the patients family members and employees right to confidentiality.
Essential Job Functions and Responsibilities:
- Assures the completion of spiritual assessment of patients/families/caregivers and that appropriate services are rendered.
- Participates in the development of an interdisciplinary plan of care to meet identified spiritual needs.
- Provides direct spiritual care to patients families and caregivers.
- Participates in interdisciplinary team meetings.
- Acts as a professional resource for consultation education and support to the interdisciplinary team on spiritual care.
- Conducts funeral or memorial service to meet the needs of families personnel and volunteers in the assigned office.
- Collaborates with community clergy and spiritual counselors as well as the interdisciplinary team. Coordinates bereavement care with the interdisciplinary team for the caregiver and family members for a period of 13 months following the death of the patient.
- Reaches across faith group boundaries and does not proselytize. Acting on behalf of the patients religious affiliation seeks to protect the patient from being confronted by other unwelcome forms of spiritual intrusion.
- Respects the religious preference of the patient.
- Must be able to lift push pull carry & transfer 10lbs.
Spiritual Care Services:
- Offers spiritual care to all patients and families.
- Performs an assessment of the spiritual needs of the patient and family within 5 days of the patients admission to hospice services.
- Spiritual assessment shall include but not be limited to the identification of any religious affiliation the patient and family may have and identify the nature and scope of any spiritual concerns or needs.
Bereavement Care Services:
- Conducts a pre- bereavement assessment of risk at the time of the patients admission.
- Assesses bereavement risk of the caregiver/family members following the patients death.
- Develops a bereavement plan of care based on the level of risk identified through assessment.
- During the six months following funeral and/or visitation attendance offers support to the bereaved utilizing the options of face-to-face visits phone calls and communication through the written word (letters and cards) as defined in the bereavement standard of care.
Hours Worked:
- Hours worked are based on the assigned patients needs.
- After-hour visits may be needed on occasions per patients needs.
Supervisor:
- Clinical Director Overseeing daytoday operations of IDT team and provision of patient care. Overseeing job performance professional development and specific role responsibilities.
Education:
- Documentation of ordination commission or credentialing according to the practices of an organized religious group.
- One unit of clinical pastoral education or has a bachelors degree with an emphasis in counseling with specific training in common spiritual issues in death and dying belief systems of comparative religions related to death and dying spiritual assessment skills: individualizing care to patient beliefs and varied spiritual practices and rituals.
License and Qualifications
- Ordination commission or credentialing in the state of Oklahoma.
- Valid drivers license.
- Current automobile insurance.
- Reliable vehicle.
