The Role: Clinical Expertise Meets Leadership
As our Clinical Care Coordinator you will be the clinical heart of our service. You will:
Lead Care Planning: Conduct home assessments and design individualised care plans that empower independence.
Mentor & Supervise: Provide specialist nursing advice and leadership to our dedicated care staff.
Hands-On Clinical Care: Manage wound care medication administration and specialised treatments.
Coordinate Continuity: Act as the vital link between clients families hospitals and allied health professionals.
Advocate: Support NDIS and My Aged Care compliance to ensure every client hits their personal goals.
