Position Summary

The Licensed Practical Nurse provides health education and care within the scope of nursing practice. As a member of a client-centered interdisciplinary team the Licensed Practical Nurse provides care to individuals promoting wellness prevention of disease and/or disability and the management of chronic diseases/conditions. The incumbent works as an advocate on behalf of the client to ensure the appropriate links/referrals to other human services that would enable the client to achieve their health goals.

Qualifications

Education
Graduate of approved Licensed Practical Nurse education program
Successful completion of Nursing Foot Care program recognized by the CLPNM.
Must be a member in good standing with CLPNM

Skills Expertise and Experience
A minimum of three (3) years experience working in an inner city community health setting
Knowledge and Experience working with Students and/or Volunteers
Current CPR Certification at Basic Rescuer Level

Proven knowledge and ability in critical thinking decision-making problem-solving and crisis management

Demonstrated ability to prioritize and focus work-related activities to effectively and efficiently achieve service goals and produce expected results/outcomes

Fluency in additional languages relevant to the client population
Experience working with service specific programs including EMR eChart and Accuro
