Job Title:Clinical Assistant Professor (C/T) in Geriatrics

Job Summary:
This faculty member in the Department of Clinical Pharmacy will have the primary responsibility of providing exemplary patient care through participating in an innovative clinical practice at the University of Colorado Hospital Seniors Clinic a geriatric primary care clinic. The primary clinic is located in the Anschutz Outpatient Pavilion on the University of Colorados Anschutz Medical Campus in Aurora CO. Support is also provided to a smaller Seniors Clinic located in Lone Tree CO. The faculty member would go to the Lone Tree location up to one day each week. The Seniors Clinic provides direct patient care within a multidisciplinary geriatric clinic model. This site is a model site for pharmacy student and pharmacy resident training and affords opportunities for scholarly work. This faculty member will collaborate with clinical faculty partners in the Seniors Clinic to provide exemplary care to patients and families further develop innovative clinical services for patients and providers and further develop this clinic as a model practice and training site.Primary clinical responsibilities include Transitional Care Management chronic disease state management using collaborative drug therapy protocols Annual Wellness Visits and dementia therapy consults. Successful candidates will also interface with clinic providers to provide comprehensive medication management and other population health initiatives for patients. The faculty member will also play an active role in the education of pharmacy students and residents as well as medical students and residents through didactic and experiential training. Successful candidates for this position are also expected to teach in the Doctor of Pharmacy curriculum in required and elective addition successful candidates for this position are expected to engage in scholarly activities that help document the value of clinical pharmacy services and improvements in patient care and outcomes in the Seniors Clinic. Finally this faculty member will provide service to the school and the pharmacy profession. The allocation of time related to this position is approximately 60-80% in clinical patient care and the remining time in academic responsibilities in teaching scholarship and service.
Key Responsibilities:
Function as an interdisciplinary member of the patient care teams to support all clinical pharmacy patient care activities in the Seniors Clinic.
Establish collaborative working relationships with clinic providers and staff to become a valued and contributing team member.
Work with clinic providers staff and administration to develop and implement clinic- and population-level medication interventions targeted to improve quality metrics.
Work collaboratively with providers clinic staff and care managers to reduce adverse drug events and improve medication management for care transitions and complex or high utilizing patients.
Educate staff and providers regarding medication management best practices including updates on new medications and clinical guidelines chronic disease state management and others.
Provide quality teaching services in the School of Pharmacy Doctor of Pharmacy program. Teaching includes preparing and presenting lectures and active learning sessions precepting student Introductory Pharmacy Practice Experiences (IPPEs) and Advanced Pharmacy Practice Experiences (APPEs) evaluating student performance using various assessment methods and mentoring and advising students.
Provide clinical instruction and mentoring to pharmacy students and residents
Participate in a scholarly program focused on clinical pharmacist activities in the Seniors Clinic. Scholarly activity may include conducting projects which document and establish the value of clinical pharmacy services and clinical interventions writing manuscripts relevant to patient care activities in the Seniors Clinic and that are published in peer-reviewed academic journals and presenting abstracts or educational presentations at professional meetings.
Participate in professional service activities such as university or school committee work participation in professional organizations and attendance at seminars conferences special courses and scientific meetings.
