University Home Care Inc is seeking a dedicated Full time Hospice Registered Nurse to join our compassionate team in Livonia MI. This role focuses on providing exceptional end-of-life care to patients and their families ensuring comfort dignity and quality of life during challenging times.

Responsibilities

• Provide skilled nursing care and symptom management to hospice patients in their homes.
• Collaborate with interdisciplinary team members to develop and implement individualized care plans.
• Educate patients and families on disease progression symptom management and available resources.
• Document patient care accurately and timely in compliance with regulatory standards.
• Respond promptly to changes in patient condition and coordinate appropriate interventions.
• Support patients and families emotionally and spiritually during end-of-life care.

Requirements

• Current and valid Registered Nurse (RN) license in the state of Michigan.
• Experience in hospice palliative care or related nursing field preferred.
• Strong clinical assessment and communication skills.
• Ability to work independently and as part of a collaborative team.
• Compassionate patient-centered approach to care.
• Reliable transportation and willingness to travel to patient homes within the Livonia area.
