Are you a compassionate Registered Nurse (RN) who finds purpose in supporting patients and families through meaningful moments of care Crescent Hospiceis seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

Schedule: Monday - Friday 8 am - 4:30 pm

Territory: Horry and Georgetown County

In this rewarding role youll deliver hands-on hospice nursing care coordinate services and guide patients and families through every step of their hospice journey. As a valued member of our interdisciplinary care team youll help ensure every patient receives The Best Care Possible care grounded in compassion clinical excellence and connection.

• Collaborative Care Champion: Partner closely with our Clinical Director to assess the eligibility and suitability of clients for our specialized hospice services. Your expertise will guide us in providing the right care at the right time.

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current drivers license vehicle insurance and access to dependable transportation.

$70000 - $80000 per year (Average Pay Range) plus $5k sign-on bonus! The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidates relevant experience qualifications and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
