JobTitle: Case Manager - Community Health

Home Office: Johnson County Office (JCO) Union County Office - Manion (UCOM) or Alexander County Office (ACO). Location is dependent on office space availability.
Area of Focus:Behavioral Health
Program(s):Southern Illinois Rural Healthcare Services Outreach program (SIRHSO) - SIRHSOfocuses on expanding access to preventative health care servicesin underserved rural areas through the following four national priority areas: Heart Disease Cancer Chronic Lower Respiratory Disease (CLRD) and Unintentional Injury. This consortium includes local healthcare providers and community-based partners who are committed to reducing health disparities and improving access to early detection and preventive services in our rural communities.Behavioral Health - Outpatient Mental Health Programming helps individuals build life skills and well-being through behavioral health counseling psychiatric care and other care coordination and support services.

Position Summary:TheCaseManagerwill provide direct support to program participants ensuring engagement with preventative services and removing barriers to accessing care.
Immediate Supervisor:SIRHSO Director - Nicole Davis

Hours of Work: Full-time 40 hours/week. Afterhours on-call rotation required.

FLSA Classification: Non-exempt
Minimum Qualifications:Bachelors Degree in Human Services or related field. Qualifies as a Mental Health Professional (MHP); or qualifies as a Licensed Practical Nurse (LPN). Minimum of three years of experience incasemanagement orhealth related support servicespreferred. Strong organizational and communication skills with experience in community outreachpreferred.

Skills & Knowledge:Knowledge of local healthcare behavioral health and community-based resources including services addressing social determinants of health such as housing food security transportation and financial assistance. Skilled in care coordination service navigation and health-related case management to support individuals in accessing medical behavioral health and supportive services. Ability to provide basic health education coaching and support to promote wellness treatment adherence and healthy behavior change. Basic crisis intervention and conflict resolution techniques. Excellent written oral visual and non-verbal communication skills. Ability to work independently and collaboratively with team members. Excellent organizational skills. Excellent time management skills and the ability to meet deadlines as assigned.
Special Requirements:Must be 21 years of age; maintains a valid drivers license and vehicle liability insurance; maintain First Aid & CPR certification; must complete required registry clearances and criminal fingerprint background checks; travel to off-site/community based locations for direct service delivery; schedules evening / or weekend hours as required; participates in after hours and day call crisis rotation.
Jobduties include but are not limited to:

  • Conduct initial participant intake and connect them with appropriate preventative services.

  • Manage acaseload of clients providing ongoing support andcasedocumentation.

  • Facilitate community education and outreach campaigns on program focus areas.

  • Collaboratewith the Registered Dietician to ensure participants have access to nutritional support.

  • Monitor participant progress and address barriers to care such as transportation or financial challenges.

  • Track referrals to healthcare providers and measure outcomes for program reporting.

  • Develop and maintain relationships with community partners to enhance participant support.

  • Assist participants in navigating healthcare systems and accessing additional resources.

  • Coordinate follow-ups with participants to ensure continuity of care and engagement in services.

  • Provide feedback to the SIRHSOAssistant Director on program challenges and opportunities for improvement.

  • Provide critical support services to program participants ensuring active engagement with grant activities and working to address barriers to successful outcomes.
  • Assist with community education and communication campaigns related to specific special focus areas including heart disease cancer chronic respiratory disease and unintentional injuries.
