Job Summary

Medical Social Worker - Hospice - Provide individualized and coordinated hospice care to bring safety and comfort to the patients and families. The holistic approach encompasses physical emotional and spiritual well-being. You will have independence and flexibility in the field to visit patients one on one in their home environment.

Hours: Full-time Days Monday - Friday and On Call Rotation

Pay and Benefits:

  • Annual Pay Range: $66040 - $81099

Territory: Our office is in Roseville MN and the Social Worker will have a general territory within our service area where they will provide in-home hospice care to the patients. The territory for this position would primarily be in the Southern section of the Twin Cities Metro area.

A few must-haves for this position:

  • Graduate of a Masters program in social work accredited by the Council on Social Work Education.
  • Licensed to practice as a Social Worker in the state of Minnesota. License is active and in good standing.
  • Minimum of one (1) years experiencein health care.
  • Hospice experience preferred
  • Able to travel within geographic area serviced by office. Valid drivers license and proof of auto insurance are required.

Some functions of what you would do as a Medical Social Worker

  • Conducts an initial psycho-social assessment of assigned patients/families within five (5) calendar days of admission to contribute to the comprehensive assessment of the patients/familys needs and the Interdisciplinary Group (IDG) plan of care.
  • Provides individual and/or family counseling in response to the mental and emotional status of the patient/family to reduce barriers to achieving the goals of care.
  • Identifies special needs related to religious or cultural diversity and how these impact the implementation of theplan of care.
  • Evaluates identified financial or environmental concerns and directs the patient/family to appropriate community resources as well as provides follow-up and acts as a liaison when crisis intervention support to the patient/family members of a facility caregiver group or the IDG.
  • Assists the patient/family in electing advance directives securing durable power of attorney making funeral arrangements and other financial legal and healthcare aspects of end-of-life choices.
