Pay: $45.00$50.00 per hour

Primary Responsibilities (including but not limited to):
- Complete all documentation for assessments plans of care reassessments and evaluations for clients
- Conduct ongoing client evaluations to ensure care remains aligned with the established plan of care
- Collaborate with management to implement and improve procedures that support high-quality client care
- Use professional clinical judgment to assess physical mental and psychosocial needs and develop appropriate care plans using a multidisciplinary approach
- Serve as a resource and mentor to team members supporting strong nursing standards and professional growth
- Advocate for clients and uphold patient rights at all times
- Identify and address home safety concerns promoting safe practices for both clients and staff
- Respond promptly to client concerns or complaints document appropriately and partner with the team to resolve issues
- Coach and reinforce proper safety procedures with caregivers and support staff

Qualifications
- Ability to travel throughout the Denver-metro area to client homes
- Able to work as a team member
- Must have a reliable transportation drivers license and car insurance
- Able to maintain confidential information
- Able to communicate effectively with the company staff clients and clients families
- Excellent verbal and communication skills adaptable in different situations and possess excellent client interaction skills
- Able to multitask and work independently

Education and Experience
- 1 year of long-term care experience preferred
- Must have current state of Colorado RN license in good standing
- One year experience in home health industry experience preferred
- Proficiency in Microsoft Outlook
- Ability to learn other software programs quickly
- Strong verbal communication skills especially using the telephone
- Ability to work independently and as part of a team
