Home Health RN Responsibilities

• Provide skilled nursing care in patient homes including assessments wound care medication management and treatments
• Develop and implement individualized home health care plans
• Monitor patient progress and adjust care plans as needed
• Educate patients and families on disease management rehab and senior care
• Coordinate care with physicians therapists and the Director of Nursing
• Document patient visits in compliance with home health regulations

Qualifications Home Health RN Jobs

• Active California Registered Nurse (RN) license (required)
• Minimum 1 year RN experience (home health experience preferred)
• Experience in rehab geriatric care or community health is a plus
• Strong clinical communication and organizational skills
• Valid drivers license and reliable transportation
• Ability to travel throughout Los Angeles County (Whittier Lakewood Santa Fe Springs)

Preferred Qualifications

• Home Health RN experience
• Public Health Nurse (PHN) certification
• Experience with mobile therapy or home-based care models
