Job Summary

OPEN JOB: Primary Care Physicians in Internal Medicine Family Medicine Emergency Medicine or Geriatrics.
LOCATION:Stockton California

Relocation Available
Regular hours of 8am-5pm Monday through Friday including home visits

BASE SALARY: $316000 to $386000

This role is to work within an award winning PACE program which allows Primary Care Physicians to provide truly comprehensive holistic care so you can focus on what really matters: your patients.

Seeking a Primary Care Physicians in Internal Medicine Family Medicine Emergency Medicine or Geriatrics.
Must be eligible for enrollment to provide services to Medicare and Medi-Cal participants.
Board certification preferred.

ROLE:

Perform comprehensive in-person assessments leading and partnering with the interdisciplinary team (IDT) to develop a personalized care plan for each participant
Coordinate discharge for acute short-term and long-term placement
No hospital rounds focus on comprehensive care seeing an average of 8 patients a day

REQUIREMENTS:

Primary Care Physicians in Internal Medicine Family Medicine Emergency Medicine or Geriatrics.
Must be eligible for enrollment to provide services to Medicare and Medi-Cal participants.
Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education
Board Certification preferred
Active state physician license without restrictions
State CDS certification and DEA registration
Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred

COMPENSATION PACKAGE:

Competitive compensation of $315900 - $386600 with quarterly bonus
Generous time off - weekends off 17 days PTO 6 sick days and 12 paid holidays
