About the job:
Pay: $42.50 - $48.00/HR
Schedule: Monday 8:00 AM-4:30 PM on-call rotation
The Clinic RN under the direction and supervision of the Clinic Supervisor provides skilled nursing care to participants implements primary care orders evaluates participant complaints in person and/or by telephone develops appropriate plan of care provides participant caregiver and family teaching and communicates participants condition to primary caregivers physician and Interdisciplinary Team.

- Graduate of a school of professional nursing
- Current state Registered Nurses (R.N.) License
- Cardio Pulmonary Resuscitation (CPR) certification required
- A minimum of one years experience in working with the frail or elderly population required
- Must have a valid drivers license proof of insurance and have means of transportation
- Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
Preferred Qualifications:
- Bachelors of Science Nursing (BSN) preferred.
Essentials:
- Conducts in-person comprehensive initial nursing assessment with new PACE enrollees. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
- Conducts an in person nursing re-assessment semi-annually and as needed.
- Functions as a member of the Interdisciplinary Team. Maintains regular attendance at and participants in Interdisciplinary Team meetings; communicates participant changes collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
- As needed provides orientation initial and annual competencies of Licensed Practical Nurse (LPN) Certified Nursing Assistants (C.N.A.s) and Clinic Clerk.
- Performs competencies on appropriate day center staff as needed.
- Performs general nursing care to participants. Performs or supervises the administration of prescribed medications and treatments in accordance with nursing standards.
