Job Summary

We are seeking a Psychiatric Nurse Practitioner to provide on-site behavioral health services within skilled nursing and extended care facilities. As the fastest growing provider of psychiatry services to the nursing home and long-term care (LTC) space in the Northeast our organization is expanding rapidly and offers providers meaningful clinical opportunity with long-term stability. This position provides a highly structured weekday schedule strong clinical autonomy and the ability to manage a consistent adult and geriatric patient panel in a non-hospital setting.

It is a financially lucrative opportunity for a provider who is comfortable practicing independently while collaborating with interdisciplinary teams. The role emphasizes long-term stabilization medication optimization and continuity of psychiatric care within a supportive care model.

Requirements


Board-certified Psychiatric Nurse Practitioner
Active unrestricted state license
Current DEA registration or ability to obtain
Strong clinical assessment and diagnostic skills
Excellent communication and interpersonal abilities
Ability to work independently while remaining team-oriented


Key Responsibilities


Conduct comprehensive psychiatric evaluations for new admissions and referred residents
Provide routine follow-up visits and ongoing medication management
Develop individualized treatment plans for mood disorders anxiety disorders psychotic disorders and neurocognitive
conditions

Prescribe monitor and adjust psychotropic medications in accordance with evidence-based standards
Address behavioral disturbances associated with dementia and other cognitive conditions
Collaborate with nursing leadership primary care providers and social services staff
Participate in care plan meetings and interdisciplinary discussions
Educate patients and family members regarding diagnoses and treatment strategies
Maintain timely accurate and compliant documentation in the electronic medical record
Ensure services meet all federal and state regulatory requirements


Preferred Experience


Experience in long-term care skilled nursing assisted living or rehabilitation settings
Background in geriatric psychiatry or chronic mental health management
Prior experience as a Psychiatric NP in institutional settings

Required Skills:

Valid Nurse Practitioner license in the state of New York with specialization in Psychiatry (PMHNP). Ability to work independently and effectively manage a caseload of patients. Strong diagnostic skills and knowledge of psychiatric medications and treatment modalities. Excellent interpersonal and communication skills to establish rapport with patients families and interdisciplinary team members. Proficient in electronic health record (EHR) systems and basic computer skills. Compassionate patient-centered approach to care.
