Responsibilities
Manage the day-to-day delivery of social work services within a rehabilitation setting
Conduct comprehensive assessments of patients mental health social and financial needs
Develop and implement individualised care plans
Collaborate with multidisciplinary teams to ensure effective care coordination
Support discharge planning to ensure appropriate community and family support
Attend case conferences and team meetings as required
Refer patients and families to relevant community resources (financial housing mental health services)
Provide crisis intervention and support including end-of-life care
Advocate for patients rights and ensure their needs are met within the healthcare system
Requirements
CORU Registration (essential)
Experience in social work; clinical experience in elderly care is preferred but not essential
Strong written and verbal communication skills
Full clean drivers licence own transport and access to a smartphone
Passionate about delivering patient-centred care within a dynamic multidisciplinary environment
