Job Summary
We are seeking a skilled and compassionate Occupational Therapist with experience working with adult and geriatric populations to join a supportive adult day care facility in Lexington North Carolina. This is a great opportunity to make a meaningful impact by helping participants maintain independence improve functional abilities and enhance their overall quality of this role you will provide individualized therapy services focused on activities of daily living mobility and safety while collaborating closely with an interdisciplinary team to support each participants unique needs.
Job Responsibilities:
Conduct evaluations to assess functional abilities and determine participant needs
Develop and implement individualized treatment plans focused on ADLs and independence
Monitor progress and adjust therapy interventions to maximize outcomes
Educate participants families and staff on carryover strategies and safety techniques
Recommend and train on adaptive equipment and environmental modifications
Maintain timely and accurate documentation in accordance with facility standards
Collaborate with interdisciplinary team members to support continuity of care
Requirements
Active state license as an Occupational Therapist
Current BLS/CPR certification
At least 6 months of relevant OT experience within the past 5 years
Experience with adult day care setting is preferred
