Pay: $51.51 - $63.45/hour
MDS Coordinator Opportunity:
- MDS Coordinator is responsible for coordinating and overseeing the Resident Assessment Instrument process including Minimum Data Set 3.0 assessments Care Area Assessments and interdisciplinary care planning in accordance with CMS CDPH and federal regulation
- Must coordinate each assessment with participation of the interdisciplinary team
- collaborates with nursing therapy social services dietary activities and business office staff to ensure accurate and timely completion of assessments under OBRA and PDPM
- This role ensures clinical documentation supports MDS coding reimbursement accuracy quality measures and skilled level of care while maintaining compliance and supporting resident outcomes
MDS Coordinator Qualifications:
- Current California RN license required.
- BLS certification
- MDS Coordinator must have a minimum 2 years SNF/LTC/acute experience
- Knowledge of CMS RAI Manual PDPM and Quality Measures
- Strong communication organizational and EHR skills
