Job Summary

About the role

The role of the Treatment Coordinator is to provide individual substance use counseling and case management to clients of Casa Esperanza Inc. using a motivational counseling approach and providing individual assessment treatment planning and case management. The Treatment Coordinator is expected to perform on-call duties and assist in the coordination of admissions treatment reporting and ESM data entry. Bilingual (Spanish and English) is required. As part of the Casa Esperanza Inc. team the Treatment Coordinator will provide services in a fully integrated holistic and collaborative manner using a team-treatment approach to ensure the coordination of all client services both across Casa programs as well as with external providers. This integrated model aims to facilitate clear communication amongst all providers and eliminate duplication of efforts or conflicting approaches to care; detail necessary follow-up and assignments; and ensure wrap-around support services are in place to maximize treatment effectiveness and clinical outcomes.


What youll do

  • Coordinate intakes admissions and discharges to and from the program
  • Completes psychosocial assessments for clients in the program and makes referrals to additional mental health and developmental support services when needed
  • Conduct initial assessment for each client and generates individualized treatment and service plans based on Casa Esperanza Inc.s Clinical Logic Model
  • Provide the 12 core counseling functions to each assigned client: screening intake orientation assessment treatment planning and counseling case management referral education crisis intervention record keeping.
  • Coordinate referrals for internal and external services i.e. legal medical housing employment and training transportation and other needed services.
  • Collaborate with other agencies public organizations and external personnel to develop comprehensive network of services.
  • Work closely with clinical team to ensure client individualized treatment and service plans are completed
  • Work with clinical team to ensure clients have access to and are participating in pro-social activities
  • Facilitate group interventions as required (including Tobacco Education etc.)
  • Conduct weekly individual counseling sessions with clients.
  • Document all client encounters and progress completes necessary reports and maintains up-to-date client files
  • Participates in clinical team meetings and team meetings to address client needsincluding MDT
  • Manages crisis interventions as needed
  • Ensures urine screening are conducted for clients as needed
  • Develop aftercare plan with the client
  • Participate in weekly clinical team meeting to address client needs
  • Participate in all trainings as required
  • Meets bi-weekly with the Program Clinical Supervisor
  • Other duties as required

Qualifications

  • Ability to use Motivational Counseling/Motivational Interviewing techniques
  • Familiarity with Cognitive Behavioral Therapy
  • Demonstrated capacity to make sound clinical decisions and manage crises Valid Massachusetts Drivers License
  • Computer skills minimum Microsoft Office
  • Experience with homelessness co-occurring disorders and people living with HIV/AIDS
  • Knowledge of substance abuse field including the AA/NA 12 Step philosophy
  • Knowledge of appropriate community resources
  • Knowledge of residential treatment models Strong verbal and written communication skills
  • Crisis management skills and group intervention skills with clients
  • Ability to work as a member of a team and independently and collaboratively Must be knowledgeable of HIPAA regulations and how to assess for need of mental health hospitalization
  • Ability to meet the physical requirements of the position (see attached)
  • Ability to meet the training requirements of the position (see attached)

Education/Training required:

  • Associate or Bachelor degree required in social work psychology counseling or related field required
  • Bilingual Spanish/English required Licensed or willing to become a licensed counselor.
  • All candidates should be willing to become CADAC certified (or a reasonable equivalent) within the first year of employment.
  • Experience providing survivor-centered trauma-informed care Experience delivering strengths-based treatment
  • CPR/First Aid certified preferred
  • Computer skillsincluding knowledge of Microsoft Office preferred


Schedule

  • On-site: Monday-Friday; 9am-5pm

While performing the duties of this job the employee is regularly required to walk stoop kneel crouch or crawl and talk or hear. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include distance vision peripheral vision and depth perception.
