Job Summary

SUMMARY
The Housing Case Manager position aids individuals and families requiring assistance by performing the following duties.

SCHEDULE:
Monday through Wednesday 9a-5p Thursday 11a-7p and Friday 9a-5p plus on-call shifts. May require occasional evening or weekend hours. The Housing Case Manager is designated Essential Personnel meaning they may be required to work remotely or be onsite during weather emergencies or state/federal emergencies.


ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.


Provide ongoing case management services to Welcome Home program clients supporting long-term housing stability and self-sufficiency.

Collaborate with local service providers housing programs and community organizations to build and maintain a strong referral network for transitional and permanent housing resources.

Develop and maintain relationships with landlords leasing agents and property management agencies to expand housing opportunities for clients.

Conduct community outreach by engaging with local stakeholders attending events and promoting organizational services to enhance visibility and partnerships.

Respond to crisis situations as needed by temporarily managing a caseload of active in-house Safehouse clients.

Participate in the Point of Contact (POC) rotation providing on-call guidance and support and covering Safehouse shifts when necessary.

Completes shelter screenings to determine if candidate meets program requirements;

Interviews clients with problems such as personal and family adjustments finances employment food clothing housing and physical and mental impairments to determine nature and degree of problem; Secures information such as medical psychological and social factors contributing to clients situation and evaluates these and clients capacities;

Meets with client individually in family or in other small groups regarding plans for meeting needs and aids client to mobilize inner capacities and environmental resources to improve social functioning;

Be knowledgeable and implement the Trauma Informed Care philosophy by creating a safe therapeutic living and working environment for our clients which espouse the proactive principles of growth change and empowerment of the individual;

Maintain and report accurate and up-to-date statistics for the program.

Will work a minimum of three 8 hour or six 4 hour holiday shifts per calendar year.

Other duties as assigned by management.



SUPERVISORY RESPONSIBILITIES
Provides consultation and direction during on-call coverage of Safehouse.



SALARY/HOURLY RATE
$27.00 - $29.00 per hour with eligibility to receive additional pay when working the shift differentials.

$4.00/hour when working any day of the week between 8pm-8am.


Additional $2.00 per hour for hours worked between:

Friday 5pm to 8pm

Saturday 8am to 8pm

Sunday 8am to 8pm

QUALIFICATIONS
To perform this job successfully an individual must be able to perform each essential duty satisfactorily. EDUCATION and/or EXPERIENCE

A Bachelors degree in Social Work Human Services or a related field preferred (or equivalent experience) and Experience in case management housing services or working with vulnerable population preferably 2 years OR a combination of related field masters level education and one year job-related experience.

Strong knowledge of community resources housing systems and social service networks. Willingness and ability to attend ongoing Trauma Informed Care trainings and webinars to stay current with up to date trends in intervention.


LANGUAGE SKILLS
Ability to read and interpret documents such as safety rules operating and maintenance instructions and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization. Excellent listening skills.


CERTIFICATES LICENSES REGISTRATIONS
Current drivers license and clean driving record. Criminal background check required. Completion of SARCs 30 hour Basic Crisis Response Training.

Required Skills:

Necessary Skills: Knowledge of intensive psychosocial services such as casework group work and individual and family therapy. Ability to work under pressure within time constraints. Knowledge of and adherence to laws policies procedures and ethical standards of the counseling profession. Willing and able to attend ongoing Trauma Informed Care trainings and webinars to stay current with trends in intervention. Excellent speaking and listening skills. Requirements for Consideration: Minimum of a Bachelors Degree is required preferably within Human Services. Valid drivers license and a clean driving record within the provisions of the agencys policy. The ability to complete SARCs 30-hour Basic Crisis Response training within 3 months of the date of hire. Preferred Experience: Experience working with vulnerable populations and victims/survivors of domestic violence sexual abuse and stalking.
