We are currently recruiting an Patient Flow Coordinator (CNM Grade) for a leading nursing home in Limerick Ireland.

Key Responsibilities
- Develop and maintain effective rapport with key stakeholders
- Ensure referral admission and discharge processes are consistently followed
- Conduct comprehensive clinical assessments of patient referrals (on-site or via telephone as appropriate)
- Assume the role of Person-in-Charge in the absence of senior management in line with relevant regulations and standards
- Support the Director of Nursing in the day-to-day operational management of the facility
- Actively participate in multidisciplinary teams including risk incident and infection control committees
- Ensure appropriate delegation of care in line with nursing standards considering staff competence and scope of practice
- Maintain accountability for tasks delegated to junior staff and provide necessary oversight
- Promote and support a restraint-free environment in line with national policies and best practices
- Contribute to and implement quality and safety systems including continuous monitoring and improvement of care standards
Requirements
- Registered in the relevant division of the Register of Nurses & Midwives maintained by NMBI or eligible for registration
- Evidence of continuous professional development (CPD)
- Qualification in teaching and assessment (desirable)
- Post-registration qualification in a relevant specialty (desirable)
- Management qualification or relevant leadership training (desirable)
- In-depth knowledge of the ageing process
- Strong IT skills and competence in the use of healthcare systems
- Minimum of 5 years post-registration experience (or equivalent full-time aggregate)
- At least 2 years experience in the relevant specialty or related area
- Demonstrated clinical managerial and administrative capability
- Proven ability to effectively manage clinical and operational responsibilities
- High standard of professional suitability and clinical expertise
- Strong leadership organisational and decision-making skills
- Ability to deliver safe high-quality patient care in line with best practice
