Job Title: Social Worker - Spine Program

Social Work Services at the Mount Sinai Hospital


The Mount Sinai Hospital is a 1134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the myriad needs of the diverse populations we serve. We believe that total patient care must emphasize the physical emotional and social needs of each patient and their care partners. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907 the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in our department provides a pathway to LCSW licensure as well as access to a broad range of continuing education professional development and extra-curricular opportunities.

This social worker will be an integral part of the Mount Sinai Hospital Spine Program.

The social worker will provide psychosocial counseling and discharge planning services for both orthopedic neurosurgery spine patients throughout the Hospital (typically on 8C 8E 8W 10E 11W).



Responsibilities

Responsibilities may include pre-operative outreach and counseling to spine patients and may include other orthopedic patients. Knowledge of crisis family and systems theories is utilized in an interdisciplinary setting.

There is a strong focus on program development and patient satisfaction. The social worker will also have responsibilities that include teamwork committee meetings and participation beyond the purview of the floor assignment. The social worker is viewed as a critical member of the Mount Sinai Spine Program and its continued success. Coverage includes med/surg specialty.



Qualifications
  • NYS LMSW or LCSW.
  • Excellent psychosocial assessment and engagement skills.
  • Demonstrated ability to develop and implement safe appropriate and timely discharge plans in a demanding fast-paced environment.
  • Exceptional collaborative and communication skills.
  • Excellent clinical organizational and organizational skills.
  • Minimum one year in current position unless a Cardiac/Surgery float.
