CNA - Home Care

Position Purpose: The Certified Nurse Assistant is responsible for providing various care services to meet the personal needs support and health status of participants.

What You Will Do:

• Provide personal care assistance to maintain participants independence in activities of daily living including toileting showers eating and grooming.
• Assist participants in maintaining physical activity exercise and walking.
• Offer behavioral supervision or support including reality orientation medication reminders dietary restrictions and emotional reassurance.
• Assist with general activities such as conversing with participants recreation activities meal and snack serving and housekeeping.

Minimum Qualifications:

• High school diploma or equivalent required.
• Active certification as a Nurse Assistant in Maryland
• Valid drivers license and use of personal vehicle required for home care assignments.
• Experience in a long-term care facility or community-based geriatric program preferred.
• Minimum of one (1) year of documented experience working with a frail or elderly population or appropriate training from PACE organization upon hiring.
• Strong conflict management and customer service skills adaptable to changing work priorities.
• Proficiency with Microsoft Office suite and electronic mail/calendar systems.
• Ability to engage in physical tasks such as kneeling crouching reaching lifting sitting walking bending and lifting up to 20 pounds.
