This position leads and supports direct caregivers by influencing clinical practice and organizational processes for optimal care outcomes. The position leads at the unit or program level and across the post-acute care continuum. Responsibilities include overseeing workflow processes for RNs and MAs and working with the team to optimize patient and staff experience trauma informed care delivery and helping out with clinical tasks as needed. The position monitors practice to ensure compliance with licensing and accrediting agency standards and participates in hospital committees and the performance improvement process.
Essential Functions
Is a consistent presence at the point of care and interacts directly with nurses staff and clinical teams.
Role models the use of evidence in daily point-of-care practice.
In collaboration with leaders and educators disseminates educational topics for continuous learning and changes in practice.
Provides teaches coaches and assists with complex care delivery and in emergencies.
Collaborates with the inter-professional team regarding patient progress updates and transitions/discharge planning.
Education
Bachelors Degree Nursing and Masters Degree Nursing
Licenses and Certifications
Registered Nurse RN - State License - Generic - HR Only required
Work Experience
6-7 years Clinical nursing experience strongly preferred
1-2 years Nursing leadership experience strongly preferred
Hours
20 hours per week
* Two evening shifts per week
* Every other weekend - one 8 hr shift
* When not working the weekend shift 3 evening shifts during the week
* Virtual meeting attendance

-Assess daily risk for infection pressure ulcers pneumonia missed care or other nurse-sensitive indicators with the care team while taking steps to decrease the risk of their occurrence.
-Collaborates with the inter-professional team regarding patient progress updates and transitions/discharge planning.

Bachelors Degree Nursing required and Masters Degree Nursing preferred
