At Bethany Home we are caring people caring for peopleand were looking for a compassionate motivated Licensed Social Worker (LSW) to join our team.

In this role youll partner with residents families and the care team to support each residents emotional social and psychosocial well-being while ensuring quality care and smooth transitions.

What Youll Do:

• Complete social service assessments and develop individualized care plans
• Participate in care conferences and collaborate with the interdisciplinary team
• Coordinate admissions discharges and transfers
• Provide emotional support and guidance to residents and families
• Connect residents with community resources and services
• Maintain accurate compliant documentation
• Advocate for resident rights dignity and quality of life

What Were Looking For:

• Bachelors or Masters degree in Social Work
• Active LSW in South Dakota (or ability to obtain quickly)
• Experience in long-term care healthcare or discharge planning preferred
• Strong communication and organizational skills

• A team-first mindset and passion for senior care
