At Thome PACE the Homecare Support Coordinator plays a vital role in ensuring participants have seamless access to the medical equipment and supplies they need to support safe and effective care in the home and community. This position leads the end-to-end coordination of equipment and supply servicesbalancing efficiency compliance and compassionate support to enhance each participants quality of life.

At Thome PACE the Homecare Support Coordinator plays a vital role in ensuring participants have seamless access to the medical equipment and supplies they need to support safe and effective care in the home and community. This position leads the end-to-end coordination of equipment and supply servicesbalancing efficiency compliance and compassionate support to enhance each participants quality of life.
Key Responsibilities
- Manage all requests ordering and coordination of durable medical equipment and medical supplies for participants.
- Ensure equipment is delivered set up and functioning properly within required timelines.
- Track and monitor all issued equipment and retrieval when no longer required.
- Coordinate repairs replacements and maintenance of equipment.
- Serve as the primary liaison with contracted DME vendors.
- Maintain an organized inventory tracking system for equipment and supplies.
- Monitor DME expenditures and assist leadership in managing equipment budgets.
- Maintain accurate records for equipment orders deliveries returns and repairs.

Skills Knowledge & Expertise
- Minimum two (2) years of experience in healthcare operations medical equipment coordination or supply chain management.
- Experience working with durable medical equipment vendors.
- Experience in home health PACE long-term care or healthcare program coordination preferred.
- Associate or bachelors degree in healthcare social work business operations or related field preferred.
- CPR certification or willingness to obtain.
- Knowledge of durable medical equipment and common medical supplies.
- Technologically savvy with ability to quickly learn and navigate relevant software and tools.
