Overview
The Family Support Coordinator creates expands and coordinates innovative community based services for individuals with developmental disabilities. Partners with individuals and families to develop individualized plans that will help them to meet their goals and lead full lives as active members of their communities.
For this 37.5 hour position we offer Tuition Assistance paid leave and sick health Insurance and a 20% Discount on Childcare Services for children and grandchildren of staff.
Salary is $20-$22.56 per hour depending on experience.
Responsibilities
- Ensure families receive timely quality services
- Serve as liaison between individuals/families community support professionals and Care Mangers as needed
- Communicate regularly with individuals receiving services and/or families regarding services provided
- Support the development of Circles of Support
- Attend individuals Life Plan and COS meetings as needed and outlined by OPWDD guidance documentation to support each individual with managing their services.
- Develop Staff Action Plans with individuals/families Care Managers and community support professionals as needed ensuring that Plans are written and implemented accurately for maximum effectiveness and participant outcome achievement.
- Monitor all service documentation to ensure timely creation of monthly summaries plans reports and other required documents as well as documentation of distribution as outlined by the program agency and outside entities and required by ADMs.
- Maintain all required documentation in eVero in compliance with service ADMs and agency procedures.
- Assess individual and family staffing needs; recruit hire train ensure credentialing of staff supervise coordinate backup staffing and initiate service referrals as needed
- Support families with staff performance evaluations and Core Competencies
- Maintain active lists of staff
- Maintain confidential files for participating individuals and staff supervised
- Assume leadership for specific program areas as assigned including all areas of the Community Support Services Department.
- Assist in the organization of trainings conferences etc. relating to Community Support Services and staff development
- Promote public awareness of Rackers Community Support Services through ongoing contact in the community
- Provide leadership and direction to staff that fosters independence and positive choices.
- Facilitate and support inclusion and relationships within the community for service participants
Qualifications
Education/Certification: Bachelors degree preferred or a combination of education/experience; ability to meet approved driver criteria or hold a valid drivers license
Skills/Abilities: Excellent interpersonal and organizational skills; excellent communication skills; ability to prioritize and multitask a high volume of work
Knowledge: Understanding of developmental disabilities human development supervisory skills and knowledge of the local community helpful. Knowledge of self-direction and support brokerage services preferred.
Physical Requirements: Ability to access familys homes and other community sites and any sites that Community Support Staff visit ability to operate standard office equipment
