Based on site at our Foothill Residential Center in Spanish Fork Utah youll work directly with clients and their families in a supportive hands-on environment. What Youll Do as a Case Manager

• Complete client assessments to identify needs and strengths
• Develop individualized care plans and update them as clients progress
• Coordinate services resources and community support
• Collaborate with clinicians treatment teams and other professionals
• Monitor client progress and maintain accurate documentation
• Help clients problem-solve barriers and build essential life skills
• Communicate effectively with clients families and team members
• Approach every interaction with empathy professionalism and a client-centered mindset

Qualifications

Applicants must meet ONE of the following:

Note: Applicants under the working toward SUDC or SSW pathway must obtain their SUDC or SSW license within 24 months of hire.

• Bachelors degree with Social Service Worker (SSW) license or Substance Use Disorder Counselor (SUDC/LASUDC)
• Bachelors degree in any field with willingness to pursue SSW or SUDC/LASUDC licensure

Additional Requirements:

• Valid drivers license
• Basic understanding of Social Work principles
• Familiarity with social welfare policy and human development
• Ability to problem-solve and maintain a client-centered approach
• Spanish speakers may qualify for increased wages after testing

Employment at Wasatch Behavioral Health is contingent upon successful completion of a drug screen motor vehicle record review and criminal background check. After hire you must complete and receive a Criminal Background Clearance (BCI) from Utah State Office of Licensing. Upon receipt of a conditional offer of employment in the position you must provide an official copy of licensure or certification.
