About the role
Provide advanced Occupational Therapy assessment and evidence-based treatment for clients on the Transitional Aged Care Program in the community.
Identify patients occupational needs through standardised and functional assessments designing tailored patient-centred interventions focused on therapy discharge planning and efficient patient flow.
Manage cases effectively and independently liaising with community services healthcare providers and support networks to ensure coordinated care and appropriate referrals.
Provide specialist advice and mentorship to Occupational Therapists across St Vincents and wider networks contributing to professional development knowledge sharing and service excellence.
Lead and participate in quality improvement initiatives and service evaluations to enhance Occupational Therapy practice within the Transitional Aged Care Program.
Educate the Transitional Aged Care Team and other community groups while responsibly managing occupational therapy equipment and organisational resources.
About you
Youre a qualified and AHPRA-registered Occupational Therapist with a Full Class C Drivers License
Proven ability to work independently with minimal supervision with strong experience providing clinical supervision and leadership to OTs support staff and students.
Advanced assessment and clinical reasoning skills delivering evidence-based interventions for aged care and chronic/complex clients; extensive experience across sub-acute rehab and community settings.
Strong team collaboration skills within multidisciplinary environments applying sound clinical judgement and advanced reasoning.
Experience initiating leading and completing quality improvement and service evaluation activities with the ability to share outcomes effectively.
