What Youll Do
As a Case Manager you will:
- Guide families through the process of transitioning from unhoused to self-sufficiency.
- Conduct one-on-one meetings to assess needs develop individualized plans and track progress.
- Provide hands-on instruction in essential life skills.
- Ensure families have vital community resources.
- Advocate for residents needs and help them overcome barriers.
- Collaborate with colleagues and community partners to ensure comprehensive service delivery.
- Maintain accurate and confidential client records.
- Serve as an ambassador for Vincent Village promoting and organization.
- Contribute to the overall success of the program.
Qualifications
- A bachelors degree in social work education or a related field is preferred; however an associates degree or equivalent case management or teaching experience is required.
- Experience working with individuals and families in unhoused and diverse populations situations is preferred.
- Valid Indiana drivers license reliable transportation and required insurance coverage.
- Fluency in Spanish and English is a plus.
Pay for this position starts at $48000 annually with the potential for a $2000 bonus upon successful completion of a 90-day review.
