What Youll Do

As a Case Manager you will:

• Guide families through the process of transitioning from unhoused to self-sufficiency.
• Conduct one-on-one meetings to assess needs develop individualized plans and track progress.
• Provide hands-on instruction in essential life skills.
• Ensure families have vital community resources.
• Advocate for residents needs and help them overcome barriers.
• Collaborate with colleagues and community partners to ensure comprehensive service delivery.
• Maintain accurate and confidential client records.
• Serve as an ambassador for Vincent Village promoting and organization.
• Contribute to the overall success of the program.

Qualifications

• A bachelors degree in social work education or a related field is preferred; however an associates degree or equivalent case management or teaching experience is required.
• Experience working with individuals and families in unhoused and diverse populations situations is preferred.
• Valid Indiana drivers license reliable transportation and required insurance coverage.
• Fluency in Spanish and English is a plus.

Pay for this position starts at $48000 annually with the potential for a $2000 bonus upon successful completion of a 90-day review.

Vincent Village is building a new resident shelter. This modern shelter will houseour residents and provide you and our other skilled and dedicated staff with a twenty-first century work environment. The new shelter will provide the ultimate setting for residents and staff to begin and complete the journey to self-sufficiency.
