As a Personal Care RN heres a big-picture view of what youll do:

• Conduct initial admissions comprehensive assessments and routine reassessments of patients receiving personal care services
• Develop implement and update individualized Plans of Care in collaboration with physicians patients and caregivers
• Perform supervisory visits with home care aides to ensure care is delivered in accordance with the Plan of Care and agency standards
• Provide ongoing clinical oversight guidance and education to personal care aides and caregivers
• Evaluate aide competency and document supervisory visits per regulatory requirements
• Monitor patient condition and promptly communicate changes to physicians and interdisciplinary team members
• Provide skilled nursing services as needed and ensure all care aligns with physician orders and state regulations
• Educate patients and families on disease processes safety and care needs
• Ensure accurate timely and compliant documentation of all visits care plans and communications
• Coordinate services and collaborate with internal teams to ensure continuity and quality of care
• Participate in case conferences quality assurance activities and performance improvement initiatives
• Assist with care transitions and discharge planning as appropriate

A few must-haves for PCSS Registered Nurses:

• Graduate of an accredited nursing program and active RN license in Virginia
• Minimum of 2 years of RN experience or completion of a preceptor program
• Active CPR certification (adult child and infant)
• Knowledge of personal care services regulations and supervisory requirements preferred
• Strong clinical judgment communication and organizational skills
• Ability to manage multiple patients and priorities independently
• Valid Drivers License and reliable transportation
