This Mental Health Case Manager serves as a critical link between clinical services and housing stability. This dual-role position provides direct case management to a dedicated caseload of individuals residing in the Mack Ashland units while simultaneously providing administrative housing application support for the Specialized Residential team ensuring a seamless ethical and clinically sound transition.

Please Note: While this position is primarily community-based it provides opportunities for some remote flexibility upon the successful completion of the training/onboarding period.

Compensation Range:

The starting pay for this position is $44000 - $55200 for unlicensed candidates $49300 - $68300 for limited licensed candidates and $51000 - $72900 annual salary for fully licensed candidates and is based on non-discriminatory factors such as skills and experience.

  • Provide comprehensive case management including person centered planning to a specialized caseload of individuals.
  • Conduct regular site visits to the Mack Ashland units to monitor consumer well-being and environmental stability.
  • Coordinate with clinical teams psychiatrists and guardians to ensure all medical and mental health needs are met including both internal and external providers.
  • Assist persons served with developing independent living skills and connecting with community resources.
  • Responsible for monitoring evaluating and recording client progress with respect to treatment goals. Modify person-centered plans according to change in client status.
  • Initiate application process and provide on-going support and coordination throughout the housing process for Mack Ashland units. Act as the primary liaison for housing applications ensuring all documentation (vouchers disability verification and income statements) is accurate and submitted timely.
  • Provide crisis intervention when needed. Completion of lethality risk assessments as indicated (e.g. suicide/homicide/potential for violence/substance use) with dispositions and recommendations that are included in Individual Plan of Service (IPOS).
  • Responsible for managing assigned caseload including ensuring that all individual documentation and authorizations are up-to-date and following admission transition and discharge procedures.
  • Utilize motivational interviewing strategies and a trauma-informed approach to engage with and provide service to individuals.
  • Complete all required documentation completely accurately and in accordance with CNS
  • Meet direct service productivity and documentation standards per agency and program expectations.
  • Monitor home provider availability and eligibility criteria for Specialized Residential applications.
  • Facilitate Specialized Residential housing placements and transitions process.
  • Ensure that the CNS Healthcare standards of service are applied to interactions with individuals served guests and staff.
  • Maintain knowledge and compliance with established policies and procedures corporate compliance program code of ethics applicable federal state and local laws and regulations HIPAA standards and other regulatory programs.
  • Perform other related duties as assigned.

Required Qualifications

  • Qualified Mental Health Professional (QMHP)
    • Bachelors Degree in Social Work Sociology Counseling Psychology Criminal Justice or other Human Services field
    • Specialized training (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons receiving mental health services as part of that experience)ORone year of experience in treating or working with a person who has a mental illness
  • Proficient with computers Microsoft office products and the use of electronic health records (EHRs).
  • Working knowledge of conflict resolution crisis intervention person-centered planning and clinical compliance practices.
  • Ability to work closely with persons with mental or physical limitations.
  • Ability to contribute to an inclusive environment that recognizes the value and contributions of all persons regardless of race ethnicity national origin gender religion age marital status sexual orientation gender identity or disability.
  • Ability to write clear concise reports and the ability to formulate and implement an effective treatment plan.
  • Ability to work collaboratively and build positive working relationships.
  • Ability to communicate effectively professionally and courteously.
  • Ability to use discretion and judgment when handling matters of a sensitive or confidential nature.
  • Advanced time management problem solving customer service interpersonal and conflict resolution skills.

Preferred

  • Prior experience in a community mental health setting.
