Position Overview
We are seeking a highly organized and compassionate Licensed Vocational Nurse (LVN) to join our team as an In-Office Hospice LVN Case Manager. This role is critical in supporting the coordination and management of hospice patients by assisting with care planning documentation review communication with field staff and ensuring regulatory compliance.
This position works closely with the Director of Patient Care Services RN Case Managers physicians and the interdisciplinary team to ensure seamless patient care and timely coordination of services.
Key Responsibilities
- Assist with the coordination and management of hospice patient care from the office.
- Review clinical documentation to ensure accuracy completeness and regulatory compliance.
- Support RN Case Managers and field staff with patient care coordination and follow-up.
- Communicate with physicians pharmacies DME providers and other healthcare partners.
- Monitor patient documentation and assist with care plan updates and physician orders.
- Assist with patient admissions recertifications and discharges from a documentation and coordination standpoint.
- Maintain accurate records in the electronic medical record (EMR).
- Participate in Interdisciplinary Team (IDT) meetings and assist with documentation needs.
- Ensure timely follow-up on lab results medication orders and clinical communications.
- Support compliance with Medicare Conditions of Participation for hospice.
Qualifications
- Active California LVN License in good standing.
- Minimum 1 year nursing experience required.
- Hospice or home health experience preferred.
- Strong organizational and multitasking skills.
- Excellent communication and documentation abilities.
- Proficiency with electronic medical records (EMR) systems.
- Ability to work collaboratively in a fast-paced healthcare environment.
Compensation: $31.00 - $45.00 per hour
