Job Summary
TRANSITION PROVIDER
Summary: Grand Island Surgery Center is n need of a CRNA locum in GI.
RATE caps for Submission(STRICTLY FOLLOW)
Malpractice Must be included in rates.
Orientation $150-157.50 - half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
Hourly $300-$315 - Per hour not OT
Holiday $450-472.50 - Must be 1.5x the hourly rate. Per holiday hour no OT
Travel Requirements:
Flight: Airfare up to $750 Economy Class
Lodging: Preferred Lodging per job description; up to 1.5x GSA
Car Rental: $65 all-in daily cap
Mileage: IRS Standard Rate
Other Expenses: N/A
Schedule/Availability Requirements:
- June 01 - July 31. Daily coverage; Monday Friday; 6:00a 2:00p

Duties: Provide anesthesia
Reason for Coverage: Employed CRNA on maternity leave and one CRNA resigned.
