RN Case Manager (Field-Based) – Chicago, IL

Job Summary

We are seeking an experiencedRegistered Nurse (RN) Case Managerto support injured workers throughout theChicago metropolitan area. Thisfield-based rolefocuses on coordinating medical care collaborating with providers and employers and supportingtimely safe return-to-work outcomes. The ideal candidate is highly independent clinically strong and experienced inworkers compensation case management.

Key Responsibilities

• Independently assess injured workers medical conditions and develop coordinated treatment plans

• Collaborate with physicians therapists employers and insurance professionals to support recovery and return-to-work

• Serve as a liaison and advocate for injured workers throughout the case lifecycle

• Coordinate and attend medical appointments as needed within the Chicago area

• Recommend job duty modifications based on medical restrictions and functional capacity

• Ensure compliance with workers compensation regulations HIPAA and organizational policies

• Maintain professional communication with all stakeholders

• Prepareaccurateandtimelycase documentation and monthly progress reports

• Exercise independent clinical judgment on complex cases and evolving treatment plans

• Participate in client meetings account in-services and occasional industry events

Qualifications

• Active Registered Nurse (RN) license(Illinois or compact-eligible)

• Prior case management experiencerequired(workers compensation strongly preferred)

• Certified Case Manager (CCM) certification preferred

• Strong understanding of medical terminology and clinical practices

• Experience in hospital rehabilitation occupational health or workers comp settings

• Ability to work independently in a field-based role

• Strong time management documentation and communication skills

• Multilingual skills a plus
