About the Role
As a Social Worker you will play a vital role in delivering strengthbased personcentred practice that empowers the people we support to live fulfilled independent and selfdirected lives.
In this role you will:
Work directly with young people (typically aged 1625) and their families to undertake holistic outcomefocused assessments.
Develop strengthsbased support plans that promote wellbeing independence and personalised life opportunities.
Build effective relationships with partner agencies including education health care providers and community organisations.
Provide clear professional analysis and recommendations in line with the Care Act 2014 and related legislation.
Manage a varied caseload ensuring timely responses and highquality social work practice.
Support young people to access opportunities in employment education independent living and community participation.
Promote safeguarding identify risks and contribute to the safety and protection of vulnerable young adults.
Contribute to a learning culture within the service and uphold the values of Cornwall Council.
About You
Were looking for someone who is:
A qualified Social Worker registered with Social Work England.
Skilled in delivering personcentred strengthsbased interventions.
Passionate about supporting young people with learning disabilities autism or other complex needs.
Confident working within a multiagency environment.
Able to communicate effectively with young people families and professionals.
Skilled in analysis decisionmaking and recording to a high professional standard.
Committed to promoting independence choice inclusion and empowerment.
Organised reflective and proactive in managing caseload demands.
Please note: If you have qualified as a Social Worker within the last 2 years but not completed your Assessed and Supported Year of Employment (ASYE) we would welcome your application as a Newly Qualified Social Worker (NQSW) in order to access Cornwalls ASYE programme which runs twice yearly. You would need to apply for our NQSW adverts which are advertised separately.
