Job Title: LVN / LPN Long-Term Care (LTC)
Location: Vestal NY 13850
Duration: 16 Weeks
Shift: Day or Evening (5x8 Schedule)
- Day Shift: 06:30 AM varies
- Evening Shift: 2:30 PM 11:45 PM
- Every other weekend required
Guaranteed Hours: 40 Hours/Week
Job Summary:
We are seeking a compassionate and skilled LVN/LPN to provide quality nursing care in a long-term care and rehabilitation setting. The role involves supporting a diverse patient population including individuals with Alzheimers and Dementia while ensuring high standards of patient care and safety.
Key Responsibilities:
- Perform medication administration (med pass) and prescribed treatments
- Monitor and record patient vital signs and overall condition
- Provide care to residents in a Rehab and nursing facility
- Support patients with Alzheimers and Dementia
- Maintain accurate and timely patient documentation
- Collaborate with interdisciplinary healthcare teams
- Educate patients and families on care plans and treatments
- Follow infection control and safety protocols
Required Qualifications:
- LPN Diploma
- Active New York (NY) LVN/LPN License (required at submission no restrictions)
- BLS Certification required
- Minimum 1 year of LTC experience (flexible)
Accepted Experience:
- Skilled Nursing Facility (SNF)
- Rehabilitation
- Med-Surg or Hospital experience
Additional Requirements:
- First-time travelers accepted
- Local candidates accepted
- COVID-19 vaccination required
- Must be available to work every other weekend
Preferred Skills:
- Experience with PointClickCare EMR (not required)
Key Competencies:
- Strong patient care and observational skills
- Excellent communication and teamwork abilities
- Ability to work in a fast-paced LTC environment
- Attention to detail and accuracy in documentation
