Customer Firstare looking for motivated and dedicatedIndependence and Wellbeing Facilitatorsto join our growing team.
Your role and responsibilities
As an Independence and Wellbeing Facilitator you will be responsible for identifying the most appropriate pathway for a customer journey promoting independence and offering appropriate solutions.
In your role you will:
- advise customers and carers on their rights the options available to them and how they can be supported accurately identifying eligibility in line with current legislation and policy.
- work in line with Suffolks Heart of Care way of working and demonstrate a genuine commitment to making a positive difference for people in Suffolk.
- champion the use of Assistive Technology including Digital Care solutions to promote independence and wellbeing.
- apply a strengths-based approach in line with the Signs of Safety model.
- manage your own workload effectively using strong organisational and time management skills to prioritise work meet deadlines and contribute to team objectives.
- share knowledge and learning with colleagues supporting continuous team development.
- complete a range of assessments including (but not limited to) carers assessments digital technology referrals and referrals for packages of care.
You will need
- a demonstrated understanding and practical application of excellent customer service principles
- strong problemsolving skills with the ability to define issues systematically and make sense of information by organising it effectively
- proven teamworking skills enabling you to establish priorities and work collaboratively towards shared goals
- familiarity with relevant Suffolk services and providers or the ability to quickly develop this knowledge to connect clients with appropriate support
- strong communication skills with the ability to engage with customers who may be vulnerable distressed or under pressure in a clear tactful and empathetic manner resolving conflict where required
- the ability to work closely with internal colleagues to build and maintain effective working relationships
- excellent IT skills with confidence using digital systems and tools

The team
Customer First/The intervention and prevention team works collaboratively to support one another and ensure a consistently high standard of service is delivered. As part of your training and continued development you will work closely with colleagues across the Customer First service as well as the Independence and Wellbeing Team Team Leaders and the Team Manager ensuring you are well supported as you develop in your role.
