DescriptionSouthwestern Behavioral Healthcare is currently seeking a Part-Time Case Manager to add to our team of professionals. The Case Manager will provide community-based and clinic-based services case management skills training and development and other treatment services to an adult outpatient population.

WHAT IS THIS POSITION RESPONSIBLE FOR
- Conduct case management skills training & development and other treatment services according to Southwestern policies.
- Provide case management services to adults assigned to OP Services including but not limited to: communicating with all parties involved in the clients treatment: identifying treatment needs and facilitating all necessary referrals; coordinating and attending collateral meetings with all parties involved in the treatment process.
- Provide clinic-based and community-based skills training and development services for adult clients. Skills training and development will be determined based on individual/familys identified needs.
- Provide emergency counseling and crisis intervention for own caseload when necessary. Maintain appropriate records of services to reflect patient progress and document treatment plans and goals.
- Maintain documents and submit reports according to established time frames.
- Communicate professionally make referrals and coordinate services with community agencies.
- Must meet efficiency standards which include chart management and patient contact standards.
QualificationsMINIMUM QUALIFICATIONS:
- Bachelors degree in Social Work Psychology Sociology Criminal Justice or other related degree required.
- Experience working with adult individuals who have experienced mental health difficulties ispreferable.
- Valid Drivers License.
