Job Summary

Were seeking an Activity Aide to involve residents in a therapeutic engagement program. You will help residents with life enrichment activities and a variety of entertainment options to help keep them engaged in their recovery programs. We are seeking candidates with a strong background in sports culture music and traditional recreational activities to help bring joy structure and stimulation to those in our care.
What Youll Do
- Lead engaging group events ranging from fitness and games to music and arts
- Lead stimulating individual cognitive and memory retention activities for our long-term residents
- Maintain accurate documentation in PointClickCare (EMR system)
- Collaborate with our health care teams to individualize your approach
- You may be asked to drive and escort residents to off-site events appointments or activities utilizing our facility van

Qualifications :
Activities Aide (in other centers this may be called a Life Enrichment Coordinator)
- Prior experience as an Activity Aide in a Skilled Nursing Facility (SNF) Intermediate Care Facility (ICF) or Assisted Living Facility (ALF) is strongly preferred.
- The ideal candidate is outgoing compassionate and skilled at creating a welcoming environment that encourages resident involvement and participation.
- Experience with electronic medical records (EMR) or patient documentation systems is preferred.
- Must possess a high school diploma or equivalent (GED).
- If assigned transport (driving) responsibilities must maintain a valid drivers license and meet the organizations motor vehicle record and insurance eligibility requirements.

This center is managed by TRANSITIONAL CARE MANAGEMENT. We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area. These settings include Long-term Care Skilled Nursing (SNFs) Short-term Care Rehabilitation Intermediate Care (ICFs) and Behavioral Health Service (SMHRFs) centers.
