Your role and responsibilities

As a Senior Independence and Wellbeing Practitioner you will work as part of the Adult Social Care hospital team operating the Discharge to Assess model alongside our health partners at Ipswich Hospital.

In this role you will:

  • Work closely with health colleagues to achieve the best possible outcomes for people
  • Use digital care solutions to assess needs and support timely discharge
  • Actively contribute to the ongoing development of the service
  • Support people and their families through effective assessment planning and coordination
  • Have opportunities to progress your career including applying to the Social Work or Occupational Therapy apprenticeship schemes when available

You will need

  • At least two years experience working as an Independence and Wellbeing Practitioner.
  • Excellent communication skills to work effectively with people their families or advocates and a wide range of professionals
  • Enthusiasm for contributing to a fast-paced multi-agency approach within urgent timescales
  • Resilience and the ability to remain positive and solutions-focused when working under pressure

The team

The hospital team is based at Bluebird Community Hospital and works using the Discharge to Assess model alongside our health partners.

As a Senior Independence and Wellbeing Practitioner you will:

  • Work within a hospital-based multidisciplinary team
  • Attend ward rounds MDT meetings and discharge planning discussions
  • Advocate for people while they are in hospital ensuring their voice and outcomes remain central to decision-making
