We are seeking an experienced Hospital Pharmacist to support inpatient pharmacy operations in a fast-paced acute care setting. The ideal candidate will have strong clinical expertise in medication management patient safety and interdisciplinary collaboration with hands-on experience in hospital pharmacy workflows and EMR systems.
Key Responsibilities:
Interpret and verify physician medication orders for accuracy and clinical appropriateness
Prepare compound and dispense medications including IV admixtures chemotherapy and parenteral nutrition
Maintain accurate patient medication records using Epic EMR
Monitor drug therapies for interactions allergies contraindications and dosing accuracy
Perform pharmacokinetic dosing and therapeutic drug monitoring
Support antibiotic stewardship by reviewing culture and sensitivity data
Ensure compliance with federal state and hospital pharmacy regulations
Identify and report adverse drug reactions and medication errors
Provide drug information and clinical consultation to healthcare teams
Participate in formulary management and recommend cost-effective therapies
Contribute to medication safety initiatives and quality improvement programs
Manage controlled substances ensuring proper storage and documentation
Participate in emergency/code response situations
Support staff training education and onboarding activities
Required Qualifications:
Active Colorado Pharmacist License (CO PHA)
Graduate of an ACPE-accredited School of Pharmacy
Strong experience in hospital/inpatient pharmacy settings
Proficiency with Epic EMR (required)
BLS & ACLS certifications required
Experience with pharmacokinetics and clinical dosing
Preferred Qualifications:
Experience with Meditech (facility transitioning to Epic)
Prior involvement in code response teams
Strong background in antibiotic stewardship
Additional Requirements:
Flu vaccination required (OctoberApril no exemptions)
Must reside 75 miles away from the facility (travel requirement)
