The RegisteredNurseCaseManager provides a wide range of nursing care to patients in the home care setting. TheRN Case Manager renders professional nursing care by assessing developing implementing and evaluating home care needs.
This position may serve Mahoning Trumbull & Columbiana counties. Looking for full time and part time candidates.
Responsibilities
Work collaboratively with physicians nurse practitionersnurses therapists social workers and caregivers to deliver comprehensive and coordinated care to our patients.
Assess plan direct and evaluate each patients care plan to ensure it meets the patients needs.
Manage emergency situations in a prompt precise and professional manner.
Complete medication administration and treatment needs for all patients assigned.
Provide nursing care in accordance with applicable regulations and company policies.
Prepare and maintain accurate and complete clinical and progress notes on a timely basis
Provides supervision and oversight to patient care.
Utilize the Electronic Visit Verification system.
Prepares physician orders as applicable.
Participates in continued education programs patient care conferences and staff meetings as scheduled.
Performs nursing assessments (initial and ongoing).
Maintains excellent assessment skills and clinical knowledge and expertise.
Notifies the physician and branch management of significant changes in patient status and obtains changes in orders as necessary.
Assumes responsibility for ongoing interdisciplinary assessment of the patient.
Participates in branch on-call rotation as assigned.
Follow organizational policies and procedures regarding infection control disposal of hazardous waste and use of protective devices as evidenced by documentation.
Performs other job-related tasks and duties as assigned.
Qualifications
- Education/Experience Desired
oNursing supervisory/leadership experience preferred
oSix months experience in home care preferred.
- License Requirements
oActiveRNstate license
oValid drivers license
oCurrent CPR certification
SKILL REQUIREMENTS
- Ability to work flexible hours as required to meet identified patients needs.
- Ability to function in any home situation regardless of age race creed color sex disability or financial condition of the patient.
- Able to work independently.
- Must possess a strong commitment to the goals mission and philosophy of the organization.
- Understands the concepts of home health and the role of theRNas a member of a full discipline health care team.
